Sleep problems in breast cancer survivors 1–10 years posttreatment

Objective:
Sleep can affect quality of life (QoL) during cancer survivorship, and symptoms related to poor sleep can be exacerbated. We examined the prevalence, severity, and nature of subjective sleep complaints in women surviving stage I–III breast cancer who were 1–10 years posttreatment. We also examined the demographic, medical, physical, and psychosocial correlates of poor sleep in these women in order to identify the subgroups that may be most in need of intervention.
Method:A total of 200 patients at a comprehensive cancer center who were 1–10 years posttreatment for primary stage I–III breast cancer with no evidence of disease at the time of enrollment completed a battery of questionnaires on demographics, sleep, physical symptoms, mood, cancer-specific fears, and QoL.
Results:The women had a mean age of 57 years (SD = 10.0), with a mean of 63.3 months (SD = 28.8) of post-cancer treatment. Some 38% of these patients were identified as having poor-quality sleep. Women with poor sleep took longer to fall asleep, had more awakenings, and acquired 2 hours less sleep per night than those with good sleep. They also had a lower QoL, greater severity of pain, more concerns about health and recurrence, and increased vasomotor symptoms (p < 0.05). Daytime sleepiness and depression were found to be not significantly correlated with sleep quality.
Significance of results:Many breast cancer survivors had severe subjective insomnia, and several breast cancer survivor subgroups were identified as having members who might be most in need of sleep-improvement interventions. Addressing physical symptoms (e.g., vasomotor symptoms and pain) and providing education about the behavioral, social, environmental, and medical factors that affect sleep could result in substantial improvement in the life course of breast cancer survivors.

The user-managed inventory (UMI) is an emerging idea for enhancing the current distribution and maintenance system for emergency medical countermeasures (MCMs). It increases current capabilities for the dispensing and distribution of MCMs and enhances local/regional preparedness and resilience. In the UMI, critical MCMs, especially those in routine medical use (“dual utility”) and those that must be administered soon after an incident before outside supplies can arrive, are stored at multiple medical facilities (including medical supply or distribution networks) across the United States. The medical facilities store a sufficient cache to meet part of the surge needs but not so much that the resources expire before they would be used in the normal course of business. In an emergency, these extra supplies can be used locally to treat casualties, including evacuees from incidents in other localities. This system, which is at the interface of local/regional and federal response, provides response capacity before the arrival of supplies from the Strategic National Stockpile (SNS) and thus enhances the local/regional medical responders' ability to provide life-saving MCMs that otherwise would be delayed. The UMI can be more cost-effective than stockpiling by avoiding costs due to drug expiration, disposal of expired stockpiled supplies, and repurchase for replacement.

Effects of high altitude and exercise on plasma erythropoietin in equids

To help resolve the mechanistic bases for haematological adaptations (~28% increase in red blood cell volume) of equids to high altitude (3800 m, barometric pressure Pb, 487 mm Hg) and exercise, plasma erythropoietin concentration ([EPO]) was measured at rest and following exercise in six, moderately fit equids (four Arabians, one Quarter Horse and one Shetland Pony; four females and two males; age 9.0 ± 4.5 years (mean ± SD)). [EPO] was measured on 2 days at 225 m (i.e. ~sea level; Pb, 743 mm Hg), over the course of a 10-day altitude exposure, and then again for 2 days after return to sea level. A standard track exercise test (submaximal, speed set-to-heart rate of 110 (trot), 150 (canter), 180 (gallop) bpm) was performed 2 days pre-high-altitude exposure and on three separate days at high altitude. In addition, a maximal incremental exercise test was performed on a high-speed motor-driven treadmill at sea level and 2 days following return to sea level from high altitude. Resting [EPO] increased from 28 ± 29 at sea level to 144 ± 46 mU ml− 1 (P < 0.05) on the first day at high altitude. By day 2 at high altitude, [EPO] had returned to baseline (31 ± 24 mU ml− 1, P>0.05 vs. pre-high altitude) and did not change over the remaining 8 days at high altitude nor over the 2 days after return to sea level. [EPO] was not significantly altered by acute exercise at sea level or at 3800 m. These results indicate that [EPO] increases rapidly (though transiently) in response to hypobaric hypoxia but not to acute exercise, and that exercise does not appear to potentiate the altitude response. Thus, if any [EPO]-derived haematological adaptations to high altitude are present, these appear to result from a transient ~4-fold elevation of [EPO] rather than any sustained increase in this signalling mechanism, at least in the equid.

Background
Twin data permit decomposition of comorbidity into genetically and environmentally derived correlations. No previous twin study includes all major forms of anxiety disorder.
AimsTo estimate the degree to which genetic and environmental risk factors are shared rather than unique to dimensionally scored panic disorder, generalised anxiety disorder, phobias, obsessive–compulsive disorder and post-traumatic stress disorder.
MethodData obtained from 2801 young-adult Norwegian twins by means of the Composite International Diagnostic Interview were analysed with the Mx program.
ResultsA multivariate common factor model fitted best. The latent liability to all anxiety disorders was substantially more heritable (54%) than the individual disorders (23% to 40%). Most of the genetic effect was common to the disorders. Genes contributed just over 50% to the covariance between liabilities.
ConclusionsThe five anxiety disorders all share genetic and environmental risk factors. This has implications for the revision of the anxiety disorder section in DSM–V.

Summary
Introduction
Welfare reform legislation at both the state and national levels since the mid-1990s has transformed the U.S. cash assistance program for single parents and their children. Among the stated goals of the federal reform legislation was to “end the dependence of needy parents on government benefits by promoting job preparation, work, and marriage.” By 2000, caseloads had declined in every state from peaks in the mid-1990s, with the national caseload declining by more than one-half. Yet, whether these declines represent improvements in the long-run well-being of current and former welfare recipients remains a contentious issue. Research has shown that postreform welfare leavers have low levels of job skills and are working in jobs with low wages, few benefits, and little security.
Reforms to welfare have been associated with substantial increases in the levels of employment for recipients (Department of Health and Human Services 2000). We know that those who leave welfare are likely to be working but that their earnings are often very low and that many of them are suffering substantial hardship (Acs and Loprest 2004; Frogner, Moffitt, and Ribar, Chapter Four this volume; Parrott 1998; Tweedie, Reichert, and O'Connor 1999). In contrast to expectations, it does not appear that reforms have caused welfare caseloads to become more disadvantaged (Moffitt and Stevens 2001), although since the implementation of Temporary Assistance for Needy Families (TANF) the nonwhite proportion has increased substantially (Zedlewski and Anderson 2001).
Differential elasticity of the immature retina: A contribution to the development of the area centralis?

Differential stretch of a retinal surface with an initially uniform cell density has been repeatedly implicated as one of the developmental mechanisms that produces the topographic organization of cell density in the adult retina, notably the area centralis or visual streak versus peripheral regions. It is known that intraocular pressure is required to produce the normal conformation and thinning of the retina during development. We tested the possibility that the retina has elastic properties that might permit differential stretch in conjunction with intraocular pressure. The relative deformation of the retina containing the presumptive area centralis was compared to the deformation of peripheral retina at equivalent applied fluid displacements in 7–12-day-old cats. The peripheral retina deformed significantly more, consistent with the hypothesis that differences in the local elasticities of the developing neural retina contribute to its characteristic topographic changes. Thus, a biomechanical property of the growing eye may contribute to the mechanism by which the pattern of the visual array is sampled.

The Betz criterion for minimum induced loss is used to compute the optimal circulation distribution along the span of flapping wings in fast forward flight. In particular, we consider the case where flapping motion is used to generate both lift (weight support) and thrust. The Betz criterion is used to develop two different numerical models of flapping. In the first model, which applies to small-amplitude harmonic flapping motions, the optimality condition is reduced to a one-dimensional integral equation which we solve numerically. In the second model, which applies to large-amplitude periodic flapping motions, the optimal circulation problem is reduced to solving for the flow over an infinitely long wavy sheet translating through an inviscid fluid at rest at infinity. This three-dimensional flow problem is solved using a vortex-lattice technique. Both methods predict that the induced power required to produce thrust decreases with increasing flapping amplitude and frequency. Using the large-amplitude theory, we find that the induced power required to produce lift increases with flapping amplitude and frequency. Therefore, an optimum flapping amplitude exists when the flapping motion of wings must simultaneously produce lift and thrust.
Morphological characterization of the retinal degeneration in three strains of mice carrying the rd-3 mutation

Retinal development in 3 strains of rd-3/rd-3 mutant mice, previously shown to have different rates of degeneration, was studied using light, electron, and immunofluorescence microscopy. The time course and phenotype of the degeneration as well as details on the mechanism of massive photoreceptor cell loss are compared with other known retinal degenerations in mice. Up until postnatal day (P) 10, the retinas of all three strains (RBF, 4Bnr, In-30) develop similarly to those of pigmented and nonpigmented controls. TUNEL-positive cells appear in the outer nuclear layer (ONL) by P14, and reach a maximum in all three mutant strains around P21. Scattered rods and cones form a loose, monolayered ONL by 8 weeks in the albino RBF strain, by 10 weeks in the albino 4Bnr strain, and by 16 weeks in the pigmented In-30 strain. Though the initial degeneration begins in the central retina, there is no preferred gradient of cell death between central and peripheral photoreceptors. Rods and cones are present at all ages examined. During development, stacks of outer segments (OS) form in all three strains though they never achieve full adult lengths, and often have disorganized, atypical OS. Rod opsin is expressed in the developing OS but is redistributed into plasma membrane as OS degeneration proceeds. Retinal pigment epithelial (RPE) cells of all mutant strains contain packets of phagocytosed OS, and their apical processes associate with the distal ends of the OS. At their synaptic sites, photoreceptor terminals contain ribbons apposed to apparently normal postsynaptic triads. As photoreceptors are lost, Müller cells fill in space in the ONL but they do not appear to undergo significant hypertrophy or migration, though during the degeneration, glial fibrillary acidic protein (GFAP) expression is gradually upregulated. Macrophage-like cells are found frequently in the subretinal space after the onset of photoreceptor apoptosis. As OS disappear, the RPE apical processes revert to simple microvilli. Late in the degeneration, some RPE cells die and neighboring cells appear to flatten as if to maintain confluence. In regions of RPE cell loss that happen to lie above retina where the ONL is gone, cells of the inner nuclear layer (INL), wrapped by Müller cell processes, may front directly on Bruch's membrane.
A review of the conservation status of the endemic Pritchardia palms of Hawaii

The conservation status of 23 Hawaiian endemic palms, Pritchardia spp., is reviewed. Field survey reports, recovery plans, herbaria holdings and observations have been utilized to assess each taxon's current range and status. Eleven species are categorized as Critically Endangered, nine Endangered, two Vulnerable and one as Data Deficient when subject to the IUCN Red List criteria. Conservation management options are discussed. A large proportion of this genus is on the verge of extinction and will continue to decline in the wild without active conservation management. Recommendations involving long-term management include maintaining and protecting the existing wild populations, establishment of effective ex situ populations, reintroduction into the wild, and the establishment of procedures to deal with invasive plants and animals.
Second malignancies in early stage laryngeal carcinoma patients treated with radiotherapy

A retrospective review of 240 patients with T1/T2 squamous cell carcinomas of the larynx was performed. Seventy-two per cent had glottic primaries, 27 per cent had supraglottic tumours and one per cent had subglottic disease. Sixty-nine per cent presented with T1 disease and 31 per cent had T2 staged tumours. All patients were treated with definitive radiotherapy between 1973 and 1997.
With a median follow-up of 68 months, 68 patients (28 per cent) have developed 72 other cancers. Ten of 68 presented with synchronous primaries (15 per cent). Thirty per cent of glottic patients and 25 per cent of the supraglottic/subglottic patients developed second cancers. The most frequent second malignancy was lung cancer: 28/72 (39 per cent). Fifteen patients developed second head and neck cancers (21 per cent). Other second primary sites included oesophagus (eight), prostate (six), colorectal (five), breast (two) and others (eight). The median time from radiotherapy until the development of a second cancer was 31 months. The Kaplan-Meier survival estimate at five years was significantly less for those patients developing second cancers (55 per cent) compared to those not developing second malignancies (70 per cent), (p<0.05). The median survival from the development of a second cancer was 14 months. More died as a result of a second cancer (41 patients) than their primary laryngeal cancer (40 patients). Second cancers are common and deadly in patients with early stage laryngeal carcinoma.
Cranial neuropathy following curative chemotherapy and radiotherapy for carcinoma of the nasopharynx

Cranial nerve damage following head and neck radiotherapy is an unusual event. Cranial neuropathy following concurrent chemotherapy and radiotherapy is unreported. The authors report a case of a 54-year-old man treated with curative chemotherapy and radiotherapy for a stage III nasopharyngeal carcinoma who developed an unilateral hypoglossal nerve palsy five years after therapy. Follow-up examination and magnetic resonance imaging (MRI) show no evidence of recurrent disease. Hypoglossal nerve injury occurring after head and neck radiotherapy is an indirect effect due to progressive soft tissue fibrosis and loss of vascularity. This process develops over years leading to nerve entrapment and permanent damage. Cranial nerve palsies, including damage to the hypoglossal nerve, can develop years after therapy with no evidence of tumour recurrence. Chemotherapy and radiotherapy have improved progression-free and overall survival in advanced nasopharyngeal cancer. As more patients achieve long-term tumour control following chemotherapy and radiotherapy, we must be cognizant of potential late injury to cranial nerves.

The Sobolev approximation for supersonic flows creates an effective opacity distribution that is nonisotropic, because the line-of-sight velocity gradient is different in different directions. To better understand the importance of this phenomenon in a simplified geometry, we consider line-driven flows in the plane-parallel zero-sound-speed limit, and solve for the wind driven by radiation with an arbitrary angular distribution. One conclusion, surprising at first glance, is that the acceleration component normal to the surface is independent of both the strength and angular profile of the driving radiation field. The flow tilt and overall mass-loss rate do depend on the character of the radiation field. Also interesting is that mass loss through a surface may be generated or enhanced by irradiation that originates above the surface.
